JOB SUMMARY
General Description: Provides care management and care coordination for adults and children with complex health and serious mental illness/serious emotional disturbance (SMI/SED). Provides services in the community and in consumers' homes, under minimal supervision. Serves in an expanded health care role to collaborate with primary health care providers, specialists, members of CMHCM health care team, and consumers/families to ensure the delivery of quality, efficient, and cost-effective physical and behavioral health care services. Assesses, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the consumer's health status. Integrates evidence-based clinical guidelines, preventive guidelines, and protocols in the development of individualized care plans that are consumer focused, promoting quality and efficiency in the delivery of physical and behavioral health care. Provides targeted interventions to avoid hospitalization and emergency room visits. Coordinates care across settings and helps consumer/families and CMHCM staff understand health care options. Ensures services and supports are based on the principles of recovery and self-determination.
Responsibilities and Duties: Works individually, with CMH staff, and with the consumers identified in the targeted high risk population. Monitors physical health care interventions and helps facilitate/consult on wellness plans for all consumers. Provides initial and follow-up health assessments, educational needs, and health care plans for identified consumers. Acts as a liaison between consumers and primary health care providers. Conducts nursing and AIMS assessments. Assists with development of person centered plans with consumers. Performs necessary record keeping according to agency standards. Obtains informed consents for psychotropic medications and coordinates and administers injectable medications. Assists and conducts medication reviews and distributes printed materials. Initiates first aid treatment when necessary. Advocates for consumers. Participates in staff meetings. Coordinates and consult with primary care. Provides nursing consultation and/or in-service education to staff, residential care providers, consumers, community agencies and other health professionals. Contacts pharmacies and coordinates with primary health care. Acts as resource person for health issues by interpreting lab work and other medical data and conducts necessary follow-up activity. Monitors consumer's blood levels. May coordinate or participate in specialized programs such as the Clozaril program, Zyprexa Relprevv, etc. Serves as nursing advisor for community living facilities/AFC homes. May attend consumer's physician appointments when necessary. Assists with consumer telepsychiatry appointments and works closely with agency psychiatrists and nurse practitioners. Performs related work as assigned.
EMPLOYMENT STANDARDS
General Education and Job-Related Experience: Diploma or associate degree from a State Board of Nursing approved school having one of two types of nursing education programs and two years of experience OR possession of a Bachelor's of Science Degree in Nursing and one year of experience OR possession of a Master's Degree in Nursing. Experience working with individuals with mental illness or developmental disabilities preferred.
Licenses/Certification: Michigan nursing license and valid driver's license required. CPR certification preferred.
SCHEDULE
This is a full time position, 40 hours per week. The schedule can either be five 8-hour days Monday - Friday 8am - 4:30pm or four 10-hour days 7:30am - 5:30pm. This will be discussed further in an interview.
COMPENSATION & BENEFITS
Salary Range: $56,380 - $74,018 depending on experience.
