Case Manager - Home-Based

SCHEDULE

This is a full time position, 40 hours per week. Monday - Friday 8am - 5pm is the typical schedule, however there are times when the schedule does need to accommodate consumer needs. This can be discussed further in an interview.

Salary Range: $47,029 - $63,981 depending on experience

SUMMARY

General Description: To provide at-risk families the services necessary to increase family skills to prevent out-of-home placement of children or ensure and expedite the return of children to their families. 

Responsibilities and Duties: Provide case management and coordinate treatment delivery to individuals, families, and significant others. Provide direct services necessary to meet diverse needs of families. Provide unscheduled crisis intervention services as needed. Co-facilitate person/family centered treatment planning and coordinate treatment delivery. Plan and provide enrichment activities, provide families with community resource information and referrals, and provide in-home and community-based services. Complete monthly reports and program summaries for court and referral source. Complete necessary paperwork/documentation for each file. Schedule individual/family appointments, attend staff meetings, attend staff development activities, complete case consultation with supervisor, and maintain confidentiality. 

EMPLOYMENT STANDARDS

General Education College: Bachelor's Degree Major: Social Work, Psychology, or similar mental health specialty.

Job-Related Experience: Sufficient experience to perform the position at a satisfactory level. Three years of supervised experience in the examination, evaluation, and treatment of minors and their families is required. 

Licenses and Certifications: Michigan Social Work licensure preferred. Valid driver's license while operating vehicle on agency business. 

CONDITIONS OF EMPLOYMENT

Knowledge: Knowledge of Person-Centered Planning; the characteristics and social aspects of mental and emotional disturbances; basic principles and techniques of psycho-social assessment, treatment planning, crisis intervention, mental health therapies, behavior modification techniques, and community organization/intervention. Knowledge of legal systems, community agencies, and health, mental health, and social welfare programs. 

Skills: Computer experience with Windows environment to include clinical software. Telephone and communication skills. 

Abilities: Ability to establish and maintain rapport with each consumer; determine therapeutic needs; develop treatment/intervention plans; monitor consumer behavior to assess plan effectiveness; and modify plans as consumers needs dictate. Ability to communicate effectively both orally and in writing; adapt communication according to consumers functioning level; and establish positive and effective relationships with colleagues and members of other community agencies/programs. Ability to work as a team member; and work well in stressful and crisis situations. 

Physical Demands: Work is generally sedentary. Frequent travel is required.

Work Environment: Work is performed in typical office setting, residential homes, and other community settings and requires working effectively people under very trying or difficult circumstances.
