SCHEDULE

This is a full time position, 40 hours per week. Monday - Friday 8am - 5pm is the typical schedule, however there are times when the schedule does need to accommodate consumer needs. This will be discussed further in an interview.

Salary Range: $53,414 - $72,550 depending on experience

JOB SUMMARY

General Description: The Assertive Community Treatment (ACT) Team Lead is responsible for the supervision and coordination of the treatment team services and treatment team members. The ACT team provides services to individuals with long-term mental illness who have serious deficits in daily living, employment, interpersonal relationships and social/leisure activities who are imminently at-risk of psychiatric hospitalization. Other responsibilities include outreach services and 24-hour emergency services to identified recipient population, aggressive client outreach, and active involvement in assisting recipients in the array of services necessary to maintain and increase community integration.

Responsibilities and Duties: Coordinates and plans services of the assertive community treatment program; accepts referrals from supervisors of the counties which the program serves. Supervises and coordinates the assertive community treatment team program with other Board services to avoid duplication, overlap, and service gaps. Formulates client treatment plans and priorities of service with other agency staff. Provides updates of client progress and client needs to other appropriate Board service providers. Supervises directly or indirectly the activities of those involved in the program, ensures that work is assigned, that schedules are set, and that these are efficient program delivery systems. Partners with supervisor for clinical supervision over the ACT team. Provides outreach services to identified client population. Provides 24-hour emergency services to identified population. Offers skill training and support in daily living skills, such as money management, shopping, grooming, cooking, laundry, etc. Performs clinical assessments and reassessments. Provides ongoing monitoring of clinical mental health status and physical health status. Provides advocacy services, especially regarding access to federal entitlements, such as SSI, SSID, GA, and food stamps. Assists clients in developing meaningful daytime activities such as employment, educational programs, volunteer services, and recreational outlets. Provides emergency financial assistance. Provides transportation and helps clients learn to use public transportation system. Offers assistance in securing housing. Provides job counseling, training and procurement, and assists clients in accessing formal employment and training programs in the community. Participates in team meetings and in agency staff meetings. Initiates client contacts with other service providers, as needed. Provides assistance in applying for non-mental health services.

EMPLOYMENT STANDARDS

General Education: Bachelor's Degree is required with a major in Social Work, Psychology, Sociology, Nursing, Criminal Justice, or comparable degree. Courses in social work, professional ethics.

Job-Related Experience: Four years of experience to perform the position at a satisfactory level. Experienced and knowledgeable in dealing with mental health issues.

Licenses, etc.: Valid driver's license while operating vehicle on agency business. Michigan Social Worker license, LLBSW or LBSW preferred. Specialized ACT certification of training preferred. Possession of an automobile or dependable access to reliable transportation for frequent travel.

CONDITIONS OF EMPLOYMENT

Knowledge: Knowledge of administrative and supervision techniques. Knowledge of the theoretical and practical bases of field of training as practiced in community mental health. Knowledge of the characteristics and social aspects of mental and emotional disturbances. Knowledge of the basic principles and techniques of psychosocial assessment, treatment planning, mental health therapies, and community organization/intervention. Knowledge of various programs in health, mental health, and social welfare programs. Knowledge of the client services management function. Knowledge and skill in crisis intervention techniques.

Skills: Interpersonal skills, leadership skills, diagnostic/assessment skills and crisis intervention skills. Use of the telephone and computer skills. Oral and written communication skills. Use of an automobile.

Abilities: Ability to exercise considerable independent and mature judgment in carrying out the duties and responsibilities of the position. Ability to establish and maintain rapport with clients, determine therapeutic needs and develop treatment/intervention plans. Ability to communicate effectively and relate positively to colleagues and members of other community agencies/programs. Ability to establish relationships and deal effectively with a wide range of persons and agencies in the community. Ability to write and speak effectively.

Physical Demands: Work involves standing, stooping, bending, walking, driving, transferring, and lifting. Frequent travel is required.

Work Environment: Work is performed partly in an agency location. However, it may involve working outside the agency setting in consumer's homes, other agency locations, or the community as a whole. It will involve dealing with people under very trying or difficult circumstances.
